Since launching in 2024, CBA has served more than 18,500 older adults, completed over 2,000 clinical visits, engaged more than 40 cross-sector partners, and is building a first-of-its-kind model that integrates healthcare, social support services, and community-based engagement to improve outcomes for older adults across the South Side. 

The Fairways for Better Aging Golf Fundraiser will bring together corporate leaders, healthcare professionals, community stakeholders, philanthropists, and advocates for a day of golf, networking, and purpose-driven impact. Funds raised will directly support programs that expand access to coordinated care, reduce social isolation, increase preventative health engagement, and improve quality of life for older adults throughout our community. 

We invite your organization to join us as a sponsor at one of the following levels:
